Registration Info for Teams in the Tournament

If you have been awarded a bid to the 2005 UPA High School Western Championships, congratulations! This tournament promises to be an exciting and unforgettable weekend for you and your team. But before the fun begins, it is important that we take care of some pre-tournament business. Please read the instructions below. All teams are required to complete all administrative paperwork completely and on-time before they will be allowed to participate in the tournament. Player and Team Eligibility

Please refer to the Player and Team Guidelines at this page: http://www.upa.org/juniors/championships/index.html to make sure your players and teams comply with all UPA eligibility rules.

Due Date

All paperwork and fees must ARRIVE (not just postmarked) at UPA Headquarters by Friday, April 22 nd.

Tournament Fees

$20/player - Checks should be made out to "UPA". Please note on the check that this is the "High School Westerns tourney fee".

Paperwork

All required paperwork can be found online http://www.upa.org/resources/sanctioning/forms.shtml along with general instructions. See below for additional descriptions of the forms found on the forms web page.

Roster - All players attending the tournament must be listed on an official 2005 UPA High School Western Championships roster.

UPA Membership Dues - All players attending the tournament must be UPA members. UPA dues for Youth Memberships for 2005 are $20/person. You can pay by check (made out to "UPA") or with a credit card by following the instructions on the UPA website http://www.upa.org/members . (One-time event fees are not sufficient for participation in this event.) You can also check to see if you are a current member online, or by calling 800-UPA-GETH (472-4384).

Waivers - All players must have a valid waiver on file with the UPA. Waivers are good for the calendar year in which they were signed. The same waiver is good for minors and adults (>18), but minor waivers must also be signed by a legal guardian.

Medical Release Form - All minors (players under 18) must return a completed medical release form. This form empowers a person or group to authorize medical care for a minor while at the tournament.

Chaperone Waiver - Each attending team must have 2 chaperones who are at least 25 years old. All chaperones must fill out and return a chaperone waiver.
